Safety and Fitness Electronic Record (Company Snapshot)

The Company Snapshot is a concise electronic record of a company’s identification, size, commodity information, and safety record, including the safety rating (if any), a roadside out-of-service inspection summary, and crash information. The Company Snapshot is available via an ad-hoc query (one carrier at a time) free of charge.

DOT Medical Form (Medical Examiner's Certificate (MEC), Form MCSA-5876)

If the Medical Examiner determines that the driver examined is physically qualified to drive a commercial motor vehicle in accordance with the physical qualification standards, the Medical Examiner will complete and provide the driver with a Medical Examiner's Certificate (MEC), Form MCSA-5876. The Medical Examiner will keep a copy of the Medical Examiner's Certificate (MEC), Form MCSA-5876 on file for at least 3 years.
